Questionnaires
When you schedule a patient in Somnoware for a sleep study, the application sends the patient an email with the day and time of the sleep study and questionnaires to complete before the study. The patient clicks a link in the email to open the questionnaire and completes it on their computer or phone. When the patient completes a questionnaire, Somnoware marks it as "Completed" on the Questionnaires tab in the patient's profile.
Patients with a tablet can navigate to myforms.somnoware.com to complete the questionnaire. To sign in, the patients must enter their last name, DOB and MRN.
If a patient is unable to complete a questionnaire on their own, you can complete it with them. You can also manually enter a patient's answers from a paper copy of the questionnaire.
In the Patients module, click the Patient Profile icon.
From the Questionnaires tab, select the questionnaire.
Next to Share Document, click the More icon (
).Click Edit.
Fill out the questionnaire.
Click Submit.
